The university tutor and representative for the PCTOs listed in the catalogue:
- interacts with the admin representative at the university from the beginning of the project, to agree on procedure and forms to be completed by the school
- checks that the school requiring activation is qualified to do so
- receives the “Student’s Training Agreement” from the School tutor (completed and signed), storing it in the office inside the “Student File” (offices of admin representatives for PCTOs, see attachment)
- explains to the student (at the beginning of the activity), any specific risks involved; the student must complete and sign the Safety Form – it is the university tutor’s responsibility to attach this to the Training Agreement and store them together
- signs the Register of Presences supplied by the School to the student, whenever necessary
- completes the Register of Presences and stores it in the Student’s File
- the Student Assessment chart by host organisation, and stores it in the Student’s File.
At the end of the activity, the Student’s File will contain the following:
- Training Agreement
- Safety Form
- Register of Presences
- Student Assessment Chart by tutor
The university administrative representative (office of admin reps for PCTOs, see attachment) will send all documents to the school.
- compila la “Scheda Valutazione studente a cura della struttura ospitante” e la conserva nel fascicolo dello studente
Al termine dell’attività il fascicolo dello studente conterrà quindi i seguenti documenti:
- Patto formativo
- Modulo sicurezza
- Registro presenze
- Scheda di valutazione dello studente da parte del tutor
Il referente amministrativo della struttura (uffici dei referenti amministrativi per l’PCTO, v. allegato) provvederà a inviare tutti i documenti alla scuola.
